Confidentiality and Your Records
Rest assure, both your medical care and records are treated in
a highly confidential manner. No information will be released to an insurance company and/or third party without a written
authorization. If you would like a copy of your records, at least a 48-hour notice will be required. There may be a slight
charge for printing/mailing but faxed at no charge.
Prescriptions and Refills
Prescription requests that are received by the office are reviewed by the doctor and a script
is then sent to the pharmacy. If authorizations are required, this process may taken longer. The doctor is in surgery on Mondays
and Thursdays, at least 48 hours are required before prescriptions will be phoned in to the patient's pharmacy. Same day refills are
no longer available. Requests received after noon will be processed on the next business day and add 48 hours for it
to be returned to the pharmacy.
Billing Information
In order for us to file your insurance claim correctly, it will be necessary to make a copy of your
insurance card and driver’s license when you arrive for your appointment. Copayments and deductibles are due at the
time of service. For your convenience, we accept VISA and Mastercard. It is your responsibility to know with whom you are
currently insured.
If your insurance
coverage requires you to have a written referral/precertification from your primary physician before seeing our specialist,
ask your provider to fax it to our office
– but the referral and/or authorization must be present prior to or upon
arrival for your appointment.
New patients: we check eligibility prior to setting appointments if we haven't recieved
a referral from your primary care physician.
Agility Orthopedics participates with most insurance plans and is continuously working on expanding
the amount of insurance companies we are contracted with. We will continue to update this page as new plans are added.
Please remember, PPO plans if not on the list maybe considered out-of-network, thus the coverage maybe subject to
a higher co-pay/deductible and also the balance covered may be reduced. Our office staff is
happy to assist you in determining eligibility. Patients are responsible for deductibles, co-payments and remaining balances.
Medicare Plans
If you have Medicare or a Medicare replacement as primary, and also have supplemental or Medigap: No payment
is necessary at the time of the visit.
If you have Medicare or an equivalent replacement as primary, but no secondary
insurance:
Payment of your 20% copay is requested at the time of the visit.Disability and Workers' Compensation
HMO Plans
Patients are responsible for notifying the
office that they have an HMO and getting the referral from their PCP before coming in to see the doctor.
Workers Comp
Patients that have been injured at work
or in a personal injury situation have unique requirements. Workers' Compensation cases must include a claim number, date
of injury, adjuster name and verification of any work-related injury or condition. It is always the patient's responsibility
to complete the required accident report and return it to the physician's office to complete the claims. Workers'
Compensation and Disability Forms require additional time and personnel, please allow five working days for completion of
this paperwork, or you may ask the office to contact you by phone when the forms are ready for pick-up.
Failure to provide accurate information may result in your liability for
the entire physician's bill. You are always responsible for the balance of your bill not covered by insurance. There is no
completion charge for Workers' Compensation forms.
