Your health is our
number one concern, however, quality of care is compromised when patients miss appointments, don't follow up with their
primary care physicians as recommended, miss scheduled surgeries at the hospital and don't follow the physician’s
treatment.
There are both medical and legal
implications for patients who are non-compliant in their care and they will be solely responsible for the outcome - this not
only puts your health at risk which may lead to possible permanent disability but is also inconsiderate to other patients
who are waiting and could have been scheduled in that window and also hospital staff waiting for no-shows.
Patients agree to comply with the minimal compliance standards
for medical treatment listed below:
PATIENTS ARE REQUIRED TO KEEP THEIR FOLLOW UP APPOINTMENTS INCLUDING BUT NOT LIMITED
TO KEEPING APPOINTMENTS WITH PRIMARY CARE DOCTORS, PHYSICAL THERAPISTS OR SPECIALISTS YOU HAVE BEEN REFERRED TO - PATIENTS
ARE REQUIRED TO CALL THE OFFICE AT LEAST 24 HOURS IN ADVANCE OF THEIR APPOINTMENT IF THEY ARE UNABLE TO MAKE IT.
PATIENTS AGREE TO COMPLY WITH THE RECOMMENDED PLAN OF CARE, FOLLOW MEDICAL INSTRUCTION
FROM THE SPECIALIST DOCTOR, YOUR PRIMARY CARE PYHSICIAN AND/OR OTHER MEDICAL ASSOCIATES AND TAKE AN ACTIVE PART IN THEIR FOLLOW-UP
CARE.
PATIENTS AGREE NOT TO ENGAGE IN ANY PRESCRIPTION ABUSE AND ANY ABUSIVE
OR THREATENING BEHAVIOR.
PARENTS AND/OR GUARDIANS OF MINORS: YOU ARE REQUIRED TO COMPLY WITH THE ABOVE AND
YOU ARE ALSO LEGALLY REQUIRED TO ACCOMPANY ANY MINORS TO OUR OFFICE AND BE IN THE ROOM FOR THE EXAMINATION.
THIS OFFICE WILL CALL TO REMIND YOU IF YOU MISSED YOUR APPOINTMENT.
FAILURE TO FOLLOW UP WILL BE CONSIDERED NON COMPLIANCE AND YOU WILL BE REQUIRED TO SEEK ORTHOPEDIC MEDICAL ATTENTION ELSEWHERE.
