Office
visits are scheduled between the hours of 9 :00 a.m. and 3:45 p.m.
There are times when there is a wait,
please be patient - it may be due to the doctor being on call, an emergency surgery, some patients requiring more time or
patients needing casting; a service which we provide for your convenience.
New patients - come in 15 minutes early to fill out the forms above, if you pick them up prior
- bring them to your office visit.
Remember to bring your insurance
card and ID with you, we check for current coverage.
When calling, please indicate the reason for your visit and the
type of insurance you have.
If you cannot keep your appointment, please let
us know 24 hours in advance.
For your convenience, we have make arrangements to have xrays done prior to your visit
- one less visit to pay for.
***Special "ortho" views are required for all knee and shoulder injuries,
let xray tech know.***
Multiple Orthopaedic Problems
Only the primary area can be evaluated per visit. For any additional body parts, a separate appointment is required
for a thorough and proper evaluation. Additional information
Should the reason you need to be treated
changes, please call immediately. This may change the length of time the doctor needs to spend with you and we may need
to reschedule your appointment. We do not want to inconvenience our other patients by making them have to wait longer than
necessary. Thank you.
If you have had MRIs or xrays ordered by another
doctor taken, please let us know and bring them with you to your visit.
What to Wear
If
you are visiting due to an upper extremity (shoulder, arm, wrist) injury, please wear or bring a tank top or sleeveless blouse.
If you are coming to our office due to a lower extremity (thigh, knee, calf, ankle) injury, please wear or bring shorts without
any metal zippers, buckles or attachments.
What to Bring
If you have MRIs or xrays for your injury, bring them with you.
New patients, please bring the new
patient forms you filled out.
Insurance ID Card(s) - Current card issued by primary and secondary (if applicable) carrier(s).
Case and employer information if injuries are Worker's Comp related.
Bring
the names of previous procedures (if any) and a list of current medications.
If you are mobility impaired, be sure to
have someone accompany you.
It is your
responsibility to check with your insurance company
prior to your appointment to see if we are contracted and if your
charges will be in-network or not.
Parking
Our office is located at 450 S. Willard St, Cottonwood AZ 86326 - across
from VVMC hospital. There is ample and convenient parking for our clients.
